Whether you're consistently struggling with back and hip pain or experiencing on-and-off soreness with tight muscles and uncomfortable joints, the last thing you need is a mattress that aggravates your existing pain. While it might not be the main cause of your discomfort, a good mattress has the potential to help alleviate it. But mattresses can be confusing — there are thousands of models available online and in stores, all marketed to help fix your back pain — and choosing the right mattress can feel daunting.

What's the best mattress for most people with back pain? While there's no one mattress that can instantly fix your chronic pain, our best overall pick is the Nolah Original 10" mattress. It constantly gets top scores in our surveys from users with back and hip pain for its pressure-relieving feel and approachable medium level of firmness. If you're on a tighter budget, we recommend the Allswell Luxe Hybrid Mattress.

The Good Housekeeping Institute Textiles Lab regularly evaluates mattresses of all kinds (including hybrid mattresses, memory foam models, latex mattresses, innerspring mattresses and more). To determine our picks, we use thorough, expertise-driven research, detailed feedback on real-world performance from consumer testers and extensive data analysis.

To find the best mattresses for back pain sufferers, we consulted medical doctors that specialize in back pain to understand how mattresses can promote pain relief. We also consulted relevant scientific literature on the topic before reviewing our proprietary mattress data. We routinely survey our tester panel for thousands of in-depth reviews on the mattresses they own. Our most recent surveys yielded responses from more than 10,000 mattress owners who answered over 170,000 questions. Unsure if you're ready for a new mattress? Pros
  • Top scores for comfort and support
  • Pressure-relieving memory foam
  • High praise from testers with joint pain
Cons
  • Testers noted it took a few days to adjust to the new mattress

For a balance between much-needed pressure relief and reliable support, Nolah's foam mattresses offer exactly what's recommended for neutral spinal alignment: a medium level of firmness. This model comes with high praise from our survey respondents and consumer testers, who swore it helped alleviate everything from tight shoulders to back and hip pain.

What it's made of: The simple foam model comes with three different components: a top layer made of comfort foam to take some tension off your joints, a supportive middle layer and a high-density base. Nolah also offers an all-foam "hybrid" variation, which features cutouts in the foam in order to replicate the responsiveness of coils.

Tester notes: While it may seem too good to be true, many users with back and hip pain have told us that this mattress made all the difference, helping notably ease discomfort. One tester noted that the bed "offers back support while still cradling pressure points," and another highlighted how the "foam bounces back to retain its support even when changing positions." On top of that, one user called it "significantly helpful in relieving back pain for [her] husband who has scoliosis."

But if you've never slept on an all-foam mattress before, allow for some time to adjust. One tester shared, "Although I was experiencing far less back, shoulder and neck pain from this mattress, it still took a few days for me to get used to."

Pros
  • Less expensive than others
  • Hybrid of foam and coils
  • Fast contact-free shipping
Cons
  • Some said it lacks edge support

If you're looking to save on your mattress, this bed-in-a-box costs much less than others, yet still gets glowing reviews from users. It only comes in one firmness option, but medium is a good choice for most people with back pain, according to multiple medical studies. Plus, the mattress arrives in a box for you to set up on your own, which many of our testers preferred because it's contactless and convenient.

What it's made of: Unlike some other boxed models that are all foam, this one is a hybrid with both foam and coils on the inside. It has 2 inches of memory foam on top, 1 inch of high-density foam for support in the middle and coils in the bottom for even more support and airflow.

Tester notes: This mattress earned high scores across the board, from ease of setup to support while sleeping. Some users even told us that it helped get rid of their back and neck pains and said it was absolutely worth the purchase. "It's great for my back," said one user, while another said, "This mattress gives much more support than my previous spring mattress." Several testers were surprised by the high-quality feel despite its low cost and compared it to a "high-end hotel bed." The caveat is that the edges sunk down more than other mattresses we've evaluated.

Pros
  • Tempur-Pedic foam at a lower price
  • Adapts to your body shape for personalized pressure relief
  • Great motion isolation, according to testers
Cons
  • 10-inch height may feel too low for some users

Tempur-Pedic's mattresses stand out for their signature, pressure-relieving feel. With superior motion isolation and contouring foam, our testers were blown away by just how comfortable they felt sleeping on this bed. And while the brand often comes at a high price, this model uses the same proprietary foam as the mattresses it sells in stores, but it's shipped directly to your door for DIY setup at a lower cost.

What's it made of: Users can choose between the popular all-foam model or pay a fee to upgrade to the hybrid version. The foam mattress uses three layers of foam, including soft, cushioning foam on top, firmer foam in the middle and a supportive bottom base. The hybrid style replaces some of the base foam with coils.

Tester notes: Our testers and survey panelists alike highlighted how the brand's foam fully cradled their hips and shoulders, especially when sleeping on their sides. They say it noticeably helped keep their spines in a relaxed, neutral alignment. This model earned especially high scores for comfort, support and perceived sleep quality. While the brand notes that the mattress may feel firm when you first lie down on it, our testers confirmed that it quickly molds to your body, providing personalized, contouring support. One tester reported, "I feel like my entire body is supported in a very gentle way."

Purple Mattress

Pros
  • Excellent breathability, according to testers
  • Pressure relief without feeling too soft
  • Grid offers motion isolation
Cons
  • Delivery took longer than expected for some

Purple stands out for its elastic grid layer at the surface which offers a combination of support and pressure relief that some users swear feels like the perfect balance of soft and firm. Aside from the unique feel, the grid structure offers excellent breathability, making this mattress a great option for those who tend to overheat at night.

What it's made of: There are two layers of foam under the elastic grid on top. The brand also has hybrid variations with a layer of foam and a layer of coils for added support and airflow. The hybrids are heavier and more expensive, but regardless, you get the standout pressure-relieving grid with all of Purple's mattresses.

Tester notes: Our panel raved about its ability to relieve their back pain with comments like, "This is the first mattress where we both woke with no pain," and, "It only took a couple of nights of sleeping on the Purple mattress for my back to feel much better!" Many have also highlighted the bed's noticeably cool feel and motion isolation, which means you're less likely to feel your sleeping partner move around at night.

Pros
  • Our panel unanimously said it's supportive
  • Ergonomically designed for spine alignment
  • Excellent customer service
Cons
  • Some say it doesn't feel cooling, despite claims
  • Testers said it ran soft

Casper has several mattresses to choose from, but this top-of-the-line model is designed for promoting proper spinal alignment. Made with an ergonomic layout, the bed provides support where you need it the most, targeting zones like the lumbar region while cradling pressure points.

What it's made of: The mattress has unique gel pods that are placed under the hips and lower back, while softer foam contours to the shape of your shoulders. Beyond that, there are several layers of foam for full-body pressure relief, a thin layer of cooling gel and perforations in the foam for breathability. Coils at the base add support and airflow.

Tester notes: Users on our panel gave it perfect ratings for support and raved about its comfort, plus they highlighted Casper's stellar customer service. One specifically called out that it "was super easy to set up and it really helps with our aches and pains." Another described it as feeling like "sleeping on a cloud." Just note that our testers found the medium firmness ran a bit soft due to the contouring, pressure-relieving nature of the foam. While the bed has cooling features, some users noted it didn't feel cool to them. So if you're a hot sleeper, consider opting for the Wave Hybrid Snow instead.

Pros
  • Choose your own firmness level
  • Dual-sided for couples
  • Earned the Good Housekeeping Seal
Cons
  • Expensive

While there isn't one mattress that can solve everyone's back pain, an adjustable mattress can provide true customizable comfort and support while automatically adapting to the shape of your body. Sleep Number's mattresses are filled with air chambers that let you customize your side of the bed to whatever firmness level you choose. It's pricey, but users told us that splurging on one was a great decision, and they wished they wouldn't have waited so long to buy it.

What it's made of: While the brand offers plenty of adjustable beds that all contain air chambers to change the firmness level, this specific model also has a 5-inch layer of foam for comfort and pressure relief. As an added bonus, there are built-in sleep tracking capabilities so you can get personalized feedback on your sleep performance sent directly to your smartphone.

Tester notes: It's definitely pricey, but Sleep Number consistently received high scores from both our testers and over 300 survey respondents. Users rated the mattress as comfortable and supportive, and especially like having personalized control over the feel of their bed. "I really feel like it gives me a custom sleep experience," noted one tester. Another told us he felt 30 years younger just days after sleeping on it. Plus, the bed is made to last. Not only has it earned the Good Housekeeping Seal, which means it's been heavily vetted by our Lab experts, but even those who've had this model for close to 10 years raved about the quality.

Avocado Green Mattress

Pros
  • Certified organic
  • Latex and coils for comfort and support
  • Year-long trial period
Cons
  • Pillow-top and in-home setup cost extra

If you're not a fan of memory foam, this mattress uses organic materials (like cotton, wool and latex) and is certified by GOTS (Global Organic Textiles Standard) and GOLS (Global Organic Latex Standard) so you know the entire manufacturing process follows strict guidelines. It's on the firmer side, but you can add a plush pillowtop (starting from $400) for a softer feel. Plus, it has a full-year trial period, which is longer than most.

What it's made of: Unlike the other mattresses in this article, this one uses latex which is firmer and has more bounce than memory foam. Below the latex is a layer of ergonomically-designed coils to support proper alignment. It also uses cotton and wool near the outer layers for comfort and temperature regulation.

Tester notes: Latex foam is known for its ability to contour to the body with a springier, more resilient feel than memory foam, and this is one of the best latex mattresses we've ever tested. Users told us they loved sleeping on this mattress, and some highlighted how it helps keep their body weight evenly distributed. One described her experience by saying, "I like that it is supportive and I don't wait up in pain as I have with other mattresses." Several even noted that it has held up well over the years without any "dips" or indents.

How we test mattresses for back pain relief

two analysts observing the construction of a mattress

The Good Housekeeping Institute Textiles Lab has a history of evaluating bedding and home products, including mattresses, using the expertise of our analysts and feedback from consumer testers and everyday users.

To find the best mattresses, our analysts reviewed the materials, design, special features and relevant specifications of various mattresses. During this process, we research all brands, including vetting certifications and consumer complaints. After narrowing down from thousands of online options, our experts and consumer testers try out mattresses firsthand for a thorough review of everything from initial setup to long-term comfort and sleep quality. Testers provide feedback after several weeks for initial review and once again after an extended period of use so we can gauge how the mattress holds up over time. On top of that, we survey our panel of Good Housekeeping readers for additional in-depth feedback on the mattresses they already own. To find the best mattresses for those with back pain, we focused on responses from users with established joint pain.

In the past few years, we've analyzed survey responses from over 10,000 mattress owners containing more than 170,000 data points and worked with consumer testers to directly review hundreds of mattresses.

Here are just some important factors we consider during our evaluations:

✔️ Ordering and delivery: Users rate and comment on the ease of purchase, arrival time and any delays or issues.

✔️ Set up: Between self-set-up and white glove delivery options, users note how easy the process was and how long it took the mattress to take shape.

✔️ Firmness level: While brands list a firmness level for mattresses, it doesn't always match up to the user experience. Testers assign a firmness rating to their mattress without knowing what the brand lists, which helps us gain insight into how soft or firm it may feel to different users.

✔️ Edge support: Testers comment on whether the bed sinks when they sit on the edges or if it holds the shape well under weight.

✔️ Comfort: Reviewers share whether they liked sleeping on the mattress immediately and after a longer adjustment period, providing both a quantitative score and more detailed comments.

✔️ Support: Similarly, users also share their thoughts on how supportive the bed feels over time.

✔️ Temperature: We ask users, including hot sleepers, whether they managed to stay at a comfortable body temperature throughout the night, which helps us gain insight into the effectiveness of cooling technology and mattress design.

✔️ Sleep quality: All respondents let us know if and how the mattress affected their sleep, from whether they woke up sore to how satisfied they are with the mattress. They are asked to compare the mattress with any other ones they've owned and to comment on the overall impact on sleep quality.

✔️ Additional feedback: Testers also share likes, dislikes and additional comments, providing helpful information to future customers.

What is the best type of mattress for back pain?

line break

Mattress shopping is tricky enough, but it's even more complicated if you're factoring in back pain. Here's what you should consider as you pick out your new bed, according to interviews with medical experts and Good Housekeeping Institute shopping pros:

  • Memory foam mattresses or hybrid mattresses are great options, according to Jaspal R. Singh, M.D., co-director of the Center for Comprehensive Spine Care at Weill Cornell Medicine and New York-Presbyterian. There isn’t one definitive type of mattress that’s best for back pain, but patients who sleep on a mattress with some type of memory foam often report a more supportive and restful sleep, since memory foam offers pressure relief, especially around the hips and shoulders.
  • For those who don't like memory foam, don't worry. While it certainly offers benefits, memory foam isn't necessarily superior to all other types of mattresses like innerspring or latex. "Patients should select mattresses that provide adequate support to their back. Memory foam vs. traditional is a matter of individual preference. Medically one is not superior to the other," says Amit Jain, M.D., M.B.A., Chief of Minimally-Invasive Spine Surgery and Associate Professor of Orthopedic Surgery at Johns Hopkins University School of Medicine.
  • Medium firmness has been shown to be effective in improving pain, according to Naimish Baxi, M.D., physiatrist at the Hospital for Special Surgery as well as a recent review of many medical studies. Plus, medium firmness offers an ideal balance of cushioning and support for side sleepers. A too-firm mattress doesn't let your shoulders or hips sink into the mattress, causing inadequate support of the neck and shoulders. On the other hand, if it’s too soft, there won’t be enough support, causing the hips and shoulders to sink in too much, resulting in improper spinal alignment.
  • A trial period is especially important for someone with back pain that struggles to find a comfortable bed. Many brands offer returns for at least 100 days so you can sleep on the mattress for a few months and get a refund if it's not a good fit. All of our recommendations have several-month-long trial periods.

What is the best sleep position for back pain?

line break

Side sleeping is often recommended if you have back pain, especially if you have pre-existing back problems. Dr. Singh even recommends side sleeping with a pillow between your knees, which will help take pressure off the lumbar spine.

That being said, the most important thing to do for back pain while sleeping is to keep your spine in neutral alignment. If you have lower back pain, sleeping on your back can “help alleviate pressure through the lower lumbar discs,” says Dr. Baxi. He also warns, “Sleeping on your stomach is generally not ideal due to the poor spine alignment when turning your head to one side.”

What is spinal alignment and why does it matter?

line break

According to Dr. Jain, "Spinal alignment refers to the ability of the head to be spatially located on top of the pelvis to minimize the energy expenditure for the body." A neutral spinal alignment can be vital, especially for those with existing pain. If sleepers spend too much energy going against the natural curve of their body, that effort can result in additional back pain. Similarly, it's crucial that your mattress accommodates the position of your spine, providing just enough comfort and support to maintain it.

Can a mattress actually get rid of back pain?

line break

It depends. Chances are, your pain is caused by an underlying issue outside of your mattress, yet buying the right mattress can certainly be one factor in getting rid of your back pain. Dr. Baxi says, “Back pain can be complicated and have many other contributing issues. For someone with chronic lower back pain, mattress characteristics can contribute to reduced pain throughout the day." In fact, he said some studies have shown that the right mattress can improve pain, stiffness and sleep quality by up to 60%.

That being said, it's a good idea to consider what else can be causing your pain. “It’s mostly related to what we do during the day that dictates the impact on our spine,” says Dr. Singh. He adds that one example is sitting at a desk all day, which puts pressure on the back and can cause lower back and neck pain.

If you find that you're experiencing recurring pain upon waking, it's best to consult with a physician to identify the nature of the pain, as a new mattress may not fully address any pre-existing conditions.
